They are not eight separate questionnaires, and this is never a clinical assessment, therapy session, or rigid interview template. They are a quiet roadmap for one natural conversation.
01 Where It All Began Childhood & Early Life The foundation of the story: earliest memories, home, family, childhood personality, friendships, routines, influences, fears, joys, and the world the person grew up in. This chapter establishes where their story began and what shaped their earliest sense of self. 02 Becoming Who I Am Adolescence & Coming of Age The transition from childhood into adulthood: teenage experiences, friendships, music, books, first jobs, independence, challenges, influences, and experiences that began shaping their identity. This chapter captures the process of becoming the person they would eventually be. 03 Finding My Way Education & Career Education, work, ambitions, career choices, unexpected turns, accomplishments, failures, mentors, colleagues, and lessons learned through working life. This chapter explores how they found their place in the world and what their work taught them about themselves and others. 04 The People I Loved Relationships & Love The relationships that mattered most: meeting a partner, falling in love, friendship, commitment, difficult seasons, enduring memories, and what they came to understand about love over time. This chapter captures the people and relationships that gave their life connection and meaning. 05 A Family of My Own Parenthood & Family Becoming a parent, raising children, family traditions, favorite memories, challenges, mistakes, hopes, and what was passed from one generation to another. This chapter explores the experience of building a family and a life with and for other people. 06 What I Came to Believe Beliefs, Values & Inner Life Faith, philosophy, values, gratitude, purpose, important principles, changing beliefs, questions about life, and the ideas that guided major decisions. This chapter reveals the inner framework behind the person's life — what they believe, value, and consider important. 07 What Life Taught Me Regrets, Resilience & Wisdom Hard seasons, adversity, mistakes, failures, forgiveness, decisions they would reconsider, personal growth, and wisdom gained through experience. This chapter gives space for the perspective that can only come from having lived through a life. 08 What I Leave Behind Legacy & Looking Back How they want to be remembered, what mattered most, what they want their family to know, words they may never have said, hopes for future generations, and the meaning they see in their life as a whole. This chapter brings the experience full circle and asks what should remain of this life for the people who come after.
